Equipment Use
WAIVER OF RELEASE AND CLAIMS ( Your waiver will be agreed to during checkout)
- The undersigned is using equipment owned by the Communication Arts Department as part of their academic requirements at Lee University. The undersigned assumes 100% responsibility for the replacement of lost, stolen, or damaged equipment. The undersigned agrees that failure to replace or pay for the lost, stolen, or damaged equipment within 30 days of loss or damage authorizes the replacement cost of the equipment to be placed on the students university bill. In order to use the Communication Arts Department equipment, the undersigned agrees to the following:
- 1. The undersigned agrees to waive and release Lee University, an independent, nonprofit corporation, and all persons and entities in interest with Lee University, including administrators, faculty members, staff members, and other employees, agents and representatives of Lee University of and from any and all claims, actions, and suits, for personal injury, death, property damage, or other loss, sustained by the undersigned in connection with the special activity above.
- 2. The undersigned agrees to abide by policies and behavioral guidelines established by Lee University and/or the equipment coordinator to be necessary for the operation of communication arts courses equipment. Check out past the return date is subject to a $5 per day fine per item. All loss and expense incurred by the undersigned's participation in this special activity, Include 100% equipment replacement, must be borne by the undersigned.
Being Responsible with Lee University Equipment
- Do not leave equipment & contents in a compromised area.
When on the set of your project be aware of your surroundings and the possibility of theft, damage, or loss of equipment.
You are responsible financially for loss or damage of equipment.
When using the equipment, CREATE A STRAIN RELIEF POINT FOR ALL POWER AND AUDIO CABLES TO PREVENT DAMAGE!
- Do not leave equipment at equipment room door when returning equipment.
Each piece of rented equipment has to be scanned in and taken off of your account.
- Do not leave equipment in your vehicle.
The temperature of your vehicle will vary and can cause the equipment to malfunction.
Also, the chance of it being stolen increases.
- Do not return equipment un-organized.
Wrap cables properly using the over/under technique for audio cables and extension cords (stingers).
Factory “bowtie” method for Lavs and power adapters.
- Treat equipment with care and respect with other students in mind.
The next person to use the equipment is depending on you to do your part in keeping the equipment operational and available.
